This initiative enables individuals who have a chronic medical condition to access up to 5 treatment sessions from allied health professionals to better manage their illness.
To be eligible for allied health services under this scheme, individuals must have a medical condition that has been present, or will be present, for six months or more and is managed by a GP under a specific management plan (called a GP Management Plan and Team Care Arrangements). Chronic medical conditions include but are not limited to asthma, cancer, cardiovascular disease, diabetes mellitus, musculoskeletal conditions and stroke.
For individuals to access psychological sessions to improve the management of an illness, your GP is required to prepare a specific management plan (called a GP Management Plan and Team Care Arrangements).
For GPs wishing to refer to our service under this initiative, patients managed by you under a GP Management Plan (MBS item number 721) and/or a Team Care Arrangements plan (MBS item number 723) can be referred to access allied health individual services.
